Any device selected over $250 must be purchased via a personal credit card and is eligible for a $250 reimbursement via Concur. The employee ordering portal and credentials will be supplied once your request has been approved by your manager.
A device can be replaced only after two years of use.

IMPORTANT: Please read the following before proceeding


  1. You will need to call your carrier and tell them that you are releasing financial responsibility of your line.
  2. Your personal mobile number cannot be the primary number on a family share plan. If it is, you will need to call your carrier and have them move it to a secondary number and make one of your other lines the primary.
  3. While on the phone with your carrier, ask them when your contract end date is and whether or not you are eligible to upgrade.
    *** = Your current contract and upgrade eligibility will NOT reset once the number is transferred to the Corporate account.

If you are requesting a new device and are NOT currently eligible to upgrade, you will need to use your current device until you are eligible to upgrade.
